Salary: $36.85 - $50.55 per hour

This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing July 18, 2023 and closing at 11:59 pm on July 25, 2023.
DUTIES & RESPONSIBILITIES:
Occupational Therapist (OT) Outreach Specialist is supported by the Patient Care Manager, Transition Management. The successful candidate will work in close collaboration with the Transition Management team, Physiotherapists and Occupational Therapists at Lakeridge and in the community, as well as with the Transitional Care Units.
The Occupational Therapy Outreach Specialist completes comprehensive in home discipline specific assessments as needed, provides consultation, education, data collection, program development, and creative problem solving related to patient care needs post hospitalization in the Transitional Care Unit Program and in the community. The position builds on the expertise of the hospital staff and other internal/external experts to improve the care planning with patients/families and community partners to ensure a smooth and timely transition.
QUALIFICATIONS:
- MSc Occupational Therapy or equivalent
- Current Registration (in good standing) with the College of Occupational Therapists of Ontario required;
- Previous clinical work in hospital and/or community
- Ability to complete in-home discipline specific assessments to be utilized in the development and execution of activation care plans in the patient home;
- Demonstrated excellence in oral and written communication;
- Excellent interpersonal, analytical thinking, problem solving and teaching skills;
- demonstrated knowledge of a wide variety of acute medical/biological issues and the required activation best-practice to support the continuity of activation care plans from hospital to community using a patient centred approach
- Ability to function collaboratively with multiple hospital interprofessional teams and with community partners for the purposes of ensuring a smooth and timely transition from hospital to the patient home, and to ensure all information regarding proven strategies/interventions are transitioned to the home setting ;
- Proven good organizations and ability to prioritize tasks
- Demonstrated desire and skill to utilize a quality improvement approach in the development of enhanced hand overs and networking between hospital and community;
- Excellent knowledge of community supports

- Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service education in this area; and,
- Proven good attendance record with capabilities of maintaining this same standard
CONDITIONS OF EMPLOYMENT:
The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.
Successful applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
