Job Description

Reports To

The Community Support Worker – Level 2 will report to the Program Director and Team Lead.

Job Overview

As a Community Support Worker-Level 2 you will provide services and supports for people living with developmental delays and other exceptionalities. Services and supports provided are based on Individual Support Plans (ISP) that you will develop with and for individuals supported. Individual Support Plans will identify the individual's personal goals, written as S.M.A.R.T. goals. As a Community Support Worker-Level 2 you will be expected to work both independently and as a part of the greater team, at times being expected to mentor and/or support other members of the team.

Responsibilities and Duties

· Complete and document a Life Skills Inventory, an Individual Support Plan, SMART goals, daily and monthly reports and any other required documentation related to the services and supports individuals have requested

· Work within the team requirements competently and respectfully; completing documentation accurately and on time, participating in team meetings, discussions and celebrations

· Understand Funding structures and work cooperatively with Finance Department to ensure documentation is accurate and complete

· Understand legislation and ensure annual Ministry targets and requirements are completed and reported in a timely manner

· Provide Monthly update and report to Leadership Team and Board of Directors

· Offer orientation, mentoring, direction and support to team members and volunteers completing/coordinating volunteer evaluations and thank you gifts

· Coordinate day to day operations of the program, both for team members and client service delivery, including team/client functions and celebrations

· Complete job responsibilities independently or with minimal direct supervision

· Consider ways and means to improve program efficiencies and service delivery, planning for future trends and challenges

· Follow the MCCSS Quality Assurance Measures and maintain all relevant documentation related to the legislation.

Qualifications

· Developmental Service Worker Diploma from a recognized Community College or a diploma in related Human Service course and/or a University Degree in a relevant course of study

· Experience relevant to the position

· Ability to work respectfully with people living with developmental disabilities and other exceptionalities

· Knowledge of other disabilities such as mental health, FASD, Autism Spectrum, dual diagnosis or others

· Knowledge and commitment to an individualized and person-centered approach

· Ability to be a positive, emotionally mature and caring role model

· Ability to problem solve

· Ability to respond to diverse support requirements

· Ability to work in a team environment that is supportive and respectful

· Ability to work professionally with the people supported, co-workers, volunteers, other service providers, professionals (behaviour therapists, speech therapists, medical doctors, dentists, nurses, social workers, etc.) and members of the community

· Ability to teach live skills, social skills and emotional intelligence skills

· Physically able to perform all aspects of the job

· Willingness to work flexible hours when required

· Willingness to work towards improving skills or learning new skills and competencies relevant to this position

Certificates Required

· Valid Ontario Driver's License and Insurance enough to carry passengers for work purposes (optional for some positions)

· Current Standard First Aid/ CPR certification

· Current Non-Violent Crisis Intervention (NVCI) certification or equivalent

· Clear Police Vulnerable Sector screening

Job Type: Full-time

Salary: $25.96 per hour
