The Interprofessional Practice and Learning team facilitates and advances person-centered care through evidence-based professional practice and learning strategies and programs that are designed provincially and implemented locally to enhance collaborative learning and practice. The Interprofessional Practice and Learning team takes a leadership role in creating a culture of collaboration with a focus on team-based care, evidence-informed practice, and learning and professional development to improve health outcomes for patients, families and communities. Through strong partnerships with the academic sector, regulators, and government, the Interprofessional Practice and Learning team has an integral role in influencing and operationalizing health system priorities.
Nova Scotia Health is leading and evolving diverse models of care in partnership with community to support access to primary care for Nova Scotians. Known as Pharmacist Walk-in Clinic+, this proof-of-concept initiative will explore the benefits of collaboration between a pharmacist and nurse practitioner to increase access to timely primary care.
Reporting to the Virtual Care Team Manager, the Nurse Practitioner (NP) provides comprehensive and coordinated care to patients as a member of a primary care team. The NP partners with and complements other members of the health care team for the achievement of person and family centred care, assessment, diagnosis, treatment, monitoring and evaluation of patient health and treatment outcomes, screening and health promotion, early detection of illness and disease, chronic disease management, continuity and coordination of care and enhanced access to programs, services and community resources.
The primary care team works together to develop and deliver services to the entire population in the community cluster, including outreach to remote areas and services to meet the needs of priority populations. This position requires providing care both virtually and in person.
The Nurse Practitioner (NP) works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform procedures within the legislated scope of practice and where required. The NP provides direct care and partners with other members of the health care team. The NP works in close collaboration with Physicians and interdisciplinary teams.
The NP adheres to the NSCN Standards for Nursing Practice and the Code of Ethics and is responsible and accountable at all times for their own practice consistent with relevant legislation, professional and ethical standards. The NP is accountable to identify, analyze and evaluate activities that are reflective of quality improvement initiatives.

Master of Nursing, Nurse Practitioner Stream required- NP License, or eligible for license, with Nova Scotia College of Nursing (NSCN)
- In the absence of candidates who meet the above qualifications, consideration may be given to candidates who hold a conditional Nurse Practitioner license with NSCN.
- Recent experience working in a Nurse Practitioner role with family of all ages preferred
- Current certification in BCLS required
- Experience with ability to organize and maintain client health records within an EMR including continuous learning around EMR functionalities considered an asset
- Experience with quality improvement initiatives and innovative work an asset
- Demonstrated ability and flexibility to work in evolving environment to meet patient population needs.
- Demonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning), care planning, case management, evaluation of client competence in self-care and other related outcomes coupled with the provision of nursing care
- Demonstrated ability to communicate effectively with patients, families, public, community partners, primary care team members and other professionals and agencies using verbal, written and computer communication means
- Demonstrated critical thinking skills within scope of practice including clinical judgment and problem solving skills, and the ability to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively
- Demonstrated leadership capabilities and strong ethical standards and a commitment to professional development and life-long learning, and ability to promote client-focused care that demonstrates care for and with clients and significant others, sensitive to diverse cultures and preferences, client advocacy and social justice concerns
- Strong understanding of health promotion, population health, and the determinants of health theory and practice
- Ability to transmit information grounded in behavior change and principals of self-management intended to instruct clients and others about topics essential to health care and well-being and demonstrated ability in applying new and innovative technologies and strategies for education and patient counselling
- Ability to manage time and resources, implementing activities to promote cooperation among relevant others, collaboration across disciplines and related activities
- Ability to promote staff morale, cooperation, assertiveness and risk-taking, creative planning for change and innovations, implementation of NSHA policies or other protocols, and ongoing professional development of self
- Ability to use factual information, prior learning and basic principles and procedures to support decisions and actions with relevant research-based evidence and integrate best practice from nursing and health-related disciplines and the humanities, arts and sciences disciplines into professional practice along with commitment to continuous learning for self, interprofessional team and supportive of interprofessional health discipline students through formal/informal mentoring opportunities
- Knowledge and experience with Microsoft Office Programs, including Outlook
- Must have valid driver’s license and access to a reliable vehicle
- Competencies in other languages an asset, French preferred, and ability to work with language agency to deliver care to patients in other languages is considered an asset

$53.08 - $62.05 hourly
$103,499 - $121,000 annually
$64.22 hourly ($125,235.82 annually) with 25 years' Nurse Practitioner Experience.
