The Interprofessional Practice and Learning team facilitates and advances person-centered care through evidence-based professional practice and learning strategies and programs that are designed provincially and implemented locally to enhance collaborative learning and practice. The Interprofessional Practice and Learning team takes a leadership role in creating a culture of collaboration with a focus on team-based care, evidence-informed practice, and learning and professional development to improve health outcomes for patients, families and communities. Through strong partnerships with the academic sector, regulators, and government, the Interprofessional Practice and Learning team has an integral role in influencing and operationalizing health system priorities.


About the Opportunity


Nova Scotia Health is leading and evolving diverse models of care in partnership with community to support access to primary care for Nova Scotians. Known as Pharmacist Walk-in Clinic+, this proof-of-concept initiative will explore the benefits of collaboration between a pharmacist and nurse practitioner to increase access to timely primary care.

Reporting to the Virtual Care Team Manager, the Nurse Practitioner (NP) provides comprehensive and coordinated care to patients as a member of a primary care team. The NP partners with and complements other members of the health care team for the achievement of person and family centred care, assessment, diagnosis, treatment, monitoring and evaluation of patient health and treatment outcomes, screening and health promotion, early detection of illness and disease, chronic disease management, continuity and coordination of care and enhanced access to programs, services and community resources.

The primary care team works together to develop and deliver services to the entire population in the community cluster, including outreach to remote areas and services to meet the needs of priority populations. This position requires providing care both virtually and in person.

The Nurse Practitioner (NP) works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform procedures within the legislated scope of practice and where required. The NP provides direct care and partners with other members of the health care team. The NP works in close collaboration with Physicians and interdisciplinary teams.

The NP adheres to the NSCN Standards for Nursing Practice and the Code of Ethics and is responsible and accountable at all times for their own practice consistent with relevant legislation, professional and ethical standards. The NP is accountable to identify, analyze and evaluate activities that are reflective of quality improvement initiatives.

Hours of Work


Permanent, Full-Time (75 hours biweekly)
Beginning April 24, 2023.
Dates are approximate and are subject to change.


Salary Information


$53.08 - $62.05 hourly
$103,499 - $121,000 annually
$64.22 hourly ($125,235.82 annually) with 25 years' Nurse Practitioner Experience.
