Location: Burns Lake, British Columbia
Employment Type:Full Time – 35 Hours/Week
Wage Range: $30.53 - $37.56 / hour

The Opportunity: The Family Preservation Worker is accountable for the day to day support and programming to individuals, families and community members. This includes the development of prevention and educational programs, one on one support and home visits to ensure families have opportunities to increase their ability to effectively care for their children and develop or maintain healthy family dynamics that promote resilience.
Perform assessments, implement goal plans and collect data and charting on progress- Develop community programming based on demonstrated need and program objectives as outlined in funding contracts, community plans and individual goal plans. Provides services to clients supported by evidence, research and multidiscipline expertise on the wide range of health, social, and program specific issues related to health care services within First Nation communities and CSFS clinics.
- Collaborates with other multidisciplinary team members and physicians to facilitate an inter-professional approach to standards of practice with a focus on promoting team-based care.
- Foster a network of support services and provide referrals as required
- Ensure cultural relativity for all services provided according to individual client needs
- Liaise with Ministry of Children and Family Social Workers, Lawyers, Community Leadership, Community Based Teams, Nurses, other Carrier Sekani Family Services Support staff and any other stakeholders to ensure client needs are met
- Maintain a documented system of records for Family Preservation Files which is inclusive of daily charting, monthly reports, quarterly reports, statistical records and reports, incident reports and the maintenance of personal files and forms required by the program

- Social Services Diploma with two years extensive experience; or
