Schedule: Monday - Friday, Standard 38.75 hour work week
Location: PCBH providers work within South Calgary family doctors' offices
Reports to: Program Manager, Primary Care Services

Our PCBH providers work independently within family doctors' offices, providing behavioural health services and support to the diverse patient population of primary care. As a PCBH team member using evidence-based methodologies, you can expect to make a difference through your work and have a direct impact on the achievement of SCPCN’s mission and vision.
As we continue to build our medical home program, our ideal candidate is a self-starter, agile, has a high degree of initiative, loves to learn, is comfortable working autonomously, and can use best judgment to make fast and efficient decisions.
Position SummaryTogether with the medical home team, the PCBH Provider will work to create a system of support and services within the patients’ medical home. These services will involve high quality, evidence-based, population-based health care to the patients, families, and communities they serve.
What Does the Role Do?
- The PCBH Provider will undergo extensive training in the PCBH model, to better identify triage, assess and provide behavioural strategies and supports to primary care patients presenting with a wide range of health and mental health concerns.
- The PCBH Provider will utilize best practice and brief evidence-based treatment interventions, including psychoeducation, motivational interviewing and cognitive behavioural strategies, to develop patient centered, behavioural change plans.
- This position involves provision of care for patients of all ages, stages and abilities, families, and groups.
- Education, consultation and collaboration with internal and external health care teams, and other referral and community groups is essential to the role.
- Other care requirements to support patient identified goals of treatment may be required, within the role of PCBH and the scope of the Registered Nurse or Social Worker. practice.
- To be successful in this role you will be required to integrate yourself into the pace and culture of Medical Home co-location, supported by colleagues both within and outside the organization.
Who Should Apply?
Bachelor’s degree in Nursing, Registered Psychiatric Nurse or Social Work.- Registration with relevant College as recognized by the Health Professions Act.
- Current Basic Life Support (BLS) for Healthcare provider.
- Minimum 2-3 years’ experience working as a Registered Nurse or Social Worker working in a community-based setting and/or psychiatric emergency roles.
- Previous experience working within a medical home/integrated care model or primary care setting.
- Previous experience using Electronic Medical Records.
- Previous experience with group development, facilitation, or instruction an asset.
- Experience and training in Primary Care Behavioral health models, health behaviour change methods and/or cognitive behavioural therapy an asset.

Working Conditions
- Working within family practice offices which may result in ergonomic challenges, limited space and resource challenges.
- Able to travel between clinics with valid driver’s license is required.
- Some evenings and weekends may be required.
- Ability to work from home may be required.
- Two supervisory references and criminal background check is required.
