We are hiring a Case Manager on our Assertive Community Treatment Team – South Simcoe

Closing Date: Aug 15, 2023


1 year contract: Full-Time, 35 Hours per Week, Evening Hours and Weekends may be required

Location: training will be available both remotely as well as onsite . There will be a combination of working from home (1-2 days per week) and working in the office/community and directly with clients (2-3 days per week). The successful candidate will be working from our Alliston Office (35 Victoria St East, Town Square Unit #1, Alliston, ON) WITH REGULAR TRAVEL IN SOUTH SIMCOE AND YORK REGION.

Job Profile:

The Case Manager works with clients 16 years of age and older who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions. S/he collaborates with the client to identify strengths and needs and develop long term goals, short term objectives to meet these goals, and concrete action steps to achieve the objectives. As part of this process individuals are assisted to acquire and use skills, and to connect with formal and informal supports, peer support, professional services, and resources to promote their recovery, improve their quality of life, and reduce the likelihood of relapse. Other duties as required.

The Case Manager collaborates with clients to provide individualized and flexible services to engage, support, and integrate clients into the community, and facilitate their use of resources and supports to maximize their independence and potential. S/he adheres to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety. The Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability.
In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.
