JOB OPPORTUNITY - Admissions and Case Coordinator, Secure Treatment Program - Full-Time Permanent
Position Summary
The Admissions and Case Coordinator position will ensure that Youthdale values are reflected in the ongoing triage of services to identified need, transition, and continued recovery of the children, youth, and young adults.
This role will assist the Admissions and Case Manager, Secure Treatment and the Acute Support Unit in intake, intake data collection and input, admissions, case management, and administrative requirements for the secure treatment program and other units as required.

Location: Inpatient Units (227 Victoria Street, Toronto, ON)
Hours: 37.5 hours per week
Status: Full-Time Permanent
Union/Non: This is a unionized position
Rate of Pay: $28.5607 per hour
Accountability
The position reports directly to: Admissions and Case Manager, Secure Treatment Program
Corporate Responsibilities

Job Responsibilities
- Assist Admissions and Case Manager, Secure Treatment in all intake functions as it relates to Court-Ordered and Emergency admissions to the ASU
- Gather collateral and referral information to present at prioritization and bed flow meetings, and facilitate a discussion to ensure acceptance decisions are made for clients who meet the CYFSA criteria for secure treatment
- Utilizes standardized tools to aid with admission/decline decision making such as the CALOCUS and INTER-RAI
- Collaborate with client and caregiver to ensure their voice is incorporated in the process
- Assist with determining the type of admission to the ASU: Court Order or Emergency admissions
- For Court Ordered admissions, assist families with the paper work required to obtain a court order, and assist with securing funding if needed
- For clients who are declined - ensure they are triaged to appropriate services
- Ensure documentation standards are met and data collection as per policy guidelines
- Provide case management services to coordinate and support appropriate services for clients and their families while at Youthdale
- Identifying, developing and coordinating appropriate discharge plans as needed for youth by collaborating with internal and external stakeholders
- Verbal communication and writing documentation that addresses client’s care and treatment. Coordinating meetings and appointments as mandates and/or needed
- Consult with inter-professional teams to determine client needs
- Working collaboratively with client and caregivers to identify and implement appropriate interventions
- Facilitates clients' utilization of appropriate community services by performing duties such as researching available services, liaising with these agencies, providing related advice and consultation to clients and providing referrals to appropriate community treatment alternatives and services. Utilizes available community resources to develop supportive and rehabilitative programs for adults and their families.
- Represent Youthdale in a professional manner during external community meetings
- Ensure standards of professional practice with respective regulatory bodies (Ontario College of Social Workers and Social Service Workers, MCCSS, MOH, Auditor, Accreditation, etc.) are met
- Maintain and adhere to all relevant legislation, organizational and occupational health and safety policies and procedures, and the mission, vision, and philosophy of the agency.
- Understanding the CYFSA and ministry licensing requirements as it relates to Child and Youth Services
- Maintain clients records and prepare reports
- Provides support and direction to clients requiring enhanced case management by monitoring their mental health status and assessing their social adjustment and ability to perform routine life activities, including managing money and taking medication regularly. Provides follow-up care and services as required, including providing referrals to other healthcare professionals such as Recreational Therapists, Occupational Therapists, and Psychiatrists.
Administration: Providing coverage to the medical transcriptionist as required for administrative related tasks regarding transcription, data collection, data sharing, data input and adherence to ministry requirements. This includes:
- Prepare, edit, and format letters, reports and summaries for physicians and upload them to the client management system (CYSIS), by pre-established deadlines
- Filing all reports in client files
- Managing faxes
- In the case of an urgent report, once file has been uploaded email/call company and let them know it is rushed and specify timeframe.
- Track all documents on tracking priority spreadsheet
- Upload/download files to/from outsourced transcription company and transcribe documents as required
- Edit and format reports and return in either printed or electronic form to the doctors for review, correction and signature for inclusion in clients’ medical records
- Review and edit transcribed reports or dictated material for spelling, grammar, clarity, consistency and proper medical terminology
- Send draft to doctor to review and make final changes, once they send back ensure that no other changes need to be made (final check)
- Once signed then Mail/fax out reports as required
- File and upload on CYSIS
Release of Information
- Documents information requests in the PHI log
- Prepares release of information package for the Privacy Officer to sign off
- Releases the package via fax or mail as appropriate.
- Scans and upload the package to CYSIS.
- Process Subpoena request as directed by Privacy Officer.
- Releases the Discharge Summaries and update the registry.
Review Board
- Provides back up coverage to Administrative Team for Review Board preparation.
- Prepare client package for Review Board requests for lawyers, as requested
- Retrieve client chart
- Scan and print documentation in client’s chart
- Notify lawyers once the package is complete
CTYS Youth Justice Intakes
- Completes intakes for CTYS YJ client’s
- Schedules appointments
- Creates digital folders for clients
- Maintains communication logs
- Creates CYSIS file
Qualifications
- College diploma in related field
- Regulated Health Care Professional and registration with the OCSWSSW, or related professional association preferred
- At least two years of experience such as intake/triage/case management
- Knowledge of community resources within the GTA and step down interventions
- Knowledge of children and youth mental health and the social determinants of health.
- Expert knowledge of system navigation and referral pathways to community mental health, addiction support, health, social agency resources, etc.
- Strong interpersonal and communication skills.
- Ability to communicate verbally and write clear, concise reports and documentation.
- Clinical experience is required to support decision-making, i.e., escalation or changes in service levels.
- Experience with multi-disciplinary collaboration/communication.
- Experience facilitating transitions between treatment services
- Experienced with standardized triage tools and assessment tools
- Some travel is required. Driver License and/or access to transportation.
