QUALIFICATIONS
- Grade 12 education (MB Standards)
- Post-secondary education in a health related field relevant to the position
- Two (2) years experience working in social services supporting clients living with substance use and mental health concerns
- Other combinations of suitable education and experience may be considered.
- Demonstrated ability to work with marginalized and culturally diverse populations
- Demonstrated ability to establish rapport with clients and families
- Self-Identify as an individual who has personal experience with the cycle of addiction
- Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology
- Knowledge of Harm Reduction principles
- Knowledge of community supports and services
- Demonstrated organizational skills, and the ability to work independently
- Demonstrated problem solving and decision making skills
- Demonstrated flexibility to facilitate changes in techniques and procedures in a changing environment
- Demonstrated knowledge and competence of skills and concepts related to the position

POSITION SUMMARY:
The Community Support Outreach Worker provides individual support to those experiencing addictions and mental health challenges. This includes identifying goals and recovery strategies, supporting persons in their treatment plans, modelling effective coping techniques, and navigating recovery support networks. The Community Support Outreach Worker will use strength based and recovery oriented approaches with individuals and/or groups to help combat stigma, promote self determination, and instill hope.
RESPONSIBILITIES:
Overview:
- Works collaboratively with interdisciplinary team to provide comprehensive services and supports to individuals.
- Orient the people they support to the outreach role including an understanding of the non-clinical nature of the role and voluntary nature of outreachsupport.
- Provide support in a manner consistent with recovery philosophy, which emphasizes
peer and natural supports, and the de-escalation of distress.
- Attend meetings such as clinical assessments when requested by an individual.
- Provides information about Prairie Mountain Health (PMH) programs and services and other community resources to individuals, and with their permission, family and significant others.
- Creates linkages to PMH programs and/or other community resources and assists clients with applications and required level of support to connect
- Provide information on individual advocacy, self-help, recovery/wellness, crisis planning and prevention, and empowerment tools as requested by the person.
- Complete statistical information and documentation as required.
- Provide outreach support services reflective of the diverse needs of the individual.
- Self-reflection, identification, authentic listening and communication of one’s own direct
experiences, for the benefit of the person they are supporting, relating to issues such as:
Living with addictions challenges, Personal experiences of recovery, Issues stemming from substance use, for example, housing, vocation, education, finances, physical health, and relationships.
- Provides a source of encouragement and hope, partnering with individuals as they move towards their self-determined goals.
- Practices the principles of anti-racism, anti-oppression.
- Builds helping relationships with individuals, supports them in identifying their strengths and in making choices which promote wellbeing and safety for individuals.
- Assists in developing recovery goals and plans with individuals.
- Assists in planning, and implementation of outreach-based programs and activities.
- Support individuals with building their social connections in the community.
- Distributes information relevant to harm reduction and enhancing well-being.
- Participates in supports and services development discussions with colleagues.
- Other duties as assigned.
