AbleLiving Services is a non-profit organization providing personal support services to adults with disabilities living within their own homes or one of our independent living facilities. Our clients direct and customize the services they receive and we support them to maximize their independence and enhance their quality of life. We provide services across the Golden Horseshoe in the Mississauga, Burlington, Stoney Creek and Hamilton communities.

POSITION SUMMARY:
Under the direction of the Coordinator, this position is responsible for helping to create effective links between appropriate health and social resources to minimize emergency room visits and hospital admissions; addressing social isolation and anchoring at risk individuals in the community. This position will adopt a holistic approach in helping clients to engage pro-actively with the community, to address social determinants of health and wellness. This position works in partnership with other stakeholders including hospital staff, primary care and other community organizations with a multi-disciplinary approach to facilitate clients transitioning from one environment to another to support living well in the community.
EMPLOYMENT GUIDE:
- University Degree in Health or Social Sciences field, or College Diploma with three (3) years’ work experience in the Health and/or Social Sciences field.
- Professional designation is an asset.
- Experience working with seniors,and extensive knowledge of the local health care system and community resources.
- Excellent interpersonal, oral and written communication skills.
- Proven ability to work as a team member and demonstrated ability to make sound judgement decision independently.
- Experience with quality improvement initiatives related to care delivery processes.
- Demonstrated initiative and experience working independently with minimal supervision.
- Proficiency with Microsoft Office programs, internet research, and ability to learn and master other computer programs as needed (i.e CareDove, Clinical Connect).
