Job Requirements
- Must be registered and hold membership in good standing as a Nurse Practitioner in Ontario
- Possession of a valid C.P.R. certification and VSC.
- Previous experience working with Addictions Medicine as a Nurse Practitioner.
- Good organization skills and ability to prioritize workload.
- Must have a valid Ontario driver's license and access to a vehicle.

Job Responsibilities
- Conduct thorough assessments of patients with addiction issues, including medical histories, physical examinations, and psychosocial evaluations.
- Develop and implement individualized care plans that incorporate harm reduction principles, evidence-based practices, and patient preferences.
- Prescribe and manage medications for addiction treatment, following regulatory guidelines and best practices.
- Monitor and evaluate patients' progress, adjusting treatment plans as necessary to optimize outcomes.
- Collaborate with the interdisciplinary team, including physicians, counselors, and social workers, to provide holistic care and support.
- Provide health education, counseling, and support to patients and their families, addressing addiction-related issues and promoting overall well-being.
- Maintain accurate and timely documentation of patient encounters, assessments, interventions, and treatment outcomes in compliance with regulatory requirements.
- Demonstrate excellent verbal and written communication skills to effectively communicate with patients, families, and team members.
- Prioritize workload effectively, managing time and resources to meet patient needs and organizational goals.
