The Toronto Grace Health Centre (TGHC) provides specialized care and services to individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, Wound Care, Respiratory Therapy, and Transitional & Remote Care. The care and services provided at TGHC helps facilitate patient flow through the health care system and ensures recovery so our patients can return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church& Bloor St), Harbour Lights (Queen & Jarvis Street) houses and a Specialized Care Centre (Victoria Park & McNicoll).

OCCUPATIONAL THERAPIST
TEMPORARY FULL TIME (18 Month Contract)
Position Summary:
The Assistive Technology Clinic (ATC) – Augmentative and Alternative Communication (AAC) Clinic at the Salvation Army, Toronto Grace Health Centre, is currently seeking a temporary full-time Occupational Therapist. ATC is an Expanded-Level AAC Clinic designated by the Ministry of Health Assistive Devices Program (ADP) to provide AAC services for individuals who have physical limitations that affect their face-to-face or written communication, including computer tasks. The Occupational Therapist assesses each client’s abilities, limitations, and communication and writing goals within the inter-disciplinary AAC team to recommend communication systems that meet the client’s daily face-to-face and/or written communication needs. The Occupational Therapist demonstrates in-depth knowledge and skills in specific areas of clinical practice and demonstrates evidence-based clinical judgement, critical thinking, and clinical reasoning. Within the vision, mission and, values of the Toronto Grace Health Centre, the Occupational Therapist assumes a leadership role in developing partnerships with patients, families and health care professionals to ensure optimal care delivery.
Responsibilities:
- As a member of an inter-disciplinary team, assesses clients who require augmentative and alternative communication (AAC) devices and/or writing aids, specifically in the areas of positioning and accessing options as they pertain to communication.
- Provides client-centred care, which is goal-directed, evidence-based, and outcome-oriented.
- Evaluates the individualized care plan through assessments and discussions with the clients their families, caregivers, and the inter-disciplinary team, and provide education and training to clients regarding their communication systems.
- Identifies, supports, and respects client and family concerns, needs, wishes and goals.
- Researches new technology, assessments, or interventions, and pursue continuous professional development.
- Prescribes communication systems in accordance with policies and procedures of the ADP – Communication Aids category, and the Centralized Equipment Pool (CEP).
- Secures third-party funding for recommended communication systems.
- Completes clinical, statistical, and program evaluation records.
- Maintains accurate and current clinical records in accordance with the College of Occupational Therapists of Ontario (COTO) and TGHC guidelines.
- Participates in supervision of students in accordance with the COTO Standards and colleges and universities.
- Participates in the development, implementation and evaluation of services/programs ensuring that services/programs are evidence-based and integrated in a coordinated manner.
- Applies critical thinking, knowledge of evidence-based practice, and leadership skills to enhance problem solving and decision making.
- Demonstrates resilience to adapt to an ever-changing environment.
- Acts as resource and advocate for clients, families and, colleagues.
- Upholds the policies and procedures of the Toronto Grace Health Centre.
- Performs other duties as assigned.
Minimum Qualifications:
- Bachelor of Science degree in Occupational Therapy from a recognized university.
- Current registration in good standing with the College of Occupational Therapists of Ontario (COTO).
- Membership in the Canadian Association of Occupational Therapists or Ontario Society of Occupational Therapists an asset.
- New graduates welcome. Previous experience in AAC is preferred. Interest in AAC and technology is a must. Training will be provided.
- Demonstrates initiative and self-direction.
- Experience with current outcome measures such as Goal Attainment Scaling.
- Good knowledge of computer hardware/software and adaptations, technology, as well as experience with seating and positioning.
- Individual Authorizer certified or awaiting IA approval from the Assistive Devices Program.
- Experience working with variety of acquired adult diagnoses, in a community based setting preferred.
- Ability to work independently in a consultative model and as a member of an inter-professional team.
- Highly motivated individual with excellent interpersonal and communication skills.
- Excellent written and verbal communication skills.
- Commitment to continuous learning, improvement and evidence-based practice.
- Demonstrates initiative and self-direction.
- Excellent attention to detail and efficient time management skills.
- Demonstrate computer skills knowledge of MS Word, Excel, Power Point and Outlook
- Excellent attendance record.
- Must have the physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position conducted by Occupational Health Services.
- Current Basic Cardiopulmonary Life Saving course preferred.
