The Job Summary:
The Waterloo Wellington (WW) Nurse Led Outreach Team Nurse Consultant will work primarily in the long term care homes in this region with strong links to hospital and community partners. The goal of this position is to work with the health care delivery team in long term care to build capacity and support knowledge uptake for clinical best practices to achieve the following: 1) the reduction of transfers to emergency departments and hospital admissions for conditions that can be treated in the long term care home setting, 2) the reduction in length of stay for residents who can be safely transferred back to the long term care setting with appropriate supports.
Key Accountabilities & Success Criteria:
- Acts in a clinical consultant role to the care team in long term care homes. This role provides assessment, consultation, education, mentorship and linkages to resources across the continuum of care in the WW region
- Works with the health care delivery team in long term care to provide required support for new referrals, admissions or follow up care.
- Supports implementation of recommendations or treatment plan established by the care team in consultation with the physician
- Develops excellent collaborative relationships with all LTC, hospital and community partners
- Is responsible for delivering education to support clinical guidelines and best practice for geriatric residents
- Collaborates with WW geriatric teams to standardize resources or develop new resources as required.
- Develops and maintains a computer inventory of available resources and promotes easy access to the resources.
- Promotes and uses clinical guidelines and evidence based practice in teaching and training.
- Maintains clinical expertise and skills needed to perform duties in an effective and efficient manner
- Participates in program development, marketing of programming, and evaluation of services delivered
- Responsible for statistics collection related to education and consultation services
- Maintains confidentiality of information as outlined in the policies of sponsoring agency and the privacy legislation
- Attends resident safety training and advocates for resident safety in daily clinical work
- Demonstrates knowledge of the health care system including acute care hospitals, community care, Long-Term Care homes and community support services
- Follows all patient and staff safety policies and procedures set by SJHCG
- Meets all requirements of SJHCG code of conduct at all times
- Demonstrates SJHCG values in all interactions
Qualifications:
Current registration with the College of Nurses of Ontario as a Registered Nurse, in good standing
- A Master’s Degree in Nursing is preferred
- Valid Ontario driver’s license with own vehicle for transportation to both Waterloo and Wellington sites for assessments, meetings, and education
Skills & Abilities:
Recent clinical skills and expertise in geriatrics and chronic disease management
- Demonstrated program development skills
- Strong communication, critical thinking and interpersonal skills
- Experience in adult learning and teaching
- Demonstrates the ability to establish a secure and efficient remote work environment that promotes productivity
- Strong knowledge of Microsoft Office suite (Word, Excel, PowerPoint, Outlook)
- Demonstrated ability to work independently, with excellent time management skills
- Demonstrated commitment to ongoing learning and professional development
