Job Description
This is an opportunity for the successful candidate to support the youth transitions team in Central Zone. This position is responsible for the short term and long term follow through with youth discharging from intensive levels of service into lower levels of care to ensure services are continuous, decreasing opportunities for breaks in the care continuum and providing service where and when needed. This role acts in a connector capacity to connect youth and families with supports if they are realized as a need once their level of care changes. This role will interface with all the programs in Red Deer, as well as liaise with practitioners across the Central Zone. The ability to build relationships, understand zonal resources internally and externally, work independently and as part of the larger zone team, and remain flexible and adaptable are key in this role. A hybrid work model may be considered for the appropriate candidate, if all conditions of hybrid work arrangement and operational needs can be met.
As a Social Worker I, you will be a member of a multi- disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families' short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.

Required Qualifications:Completion of diploma in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Experience working with youth and families exhibiting risk (ie. domestic violence, suicidality, self-harm, etc). Strong technical skills required.
Preferred Qualifications:Experience with complex youth and families considered an asset. Knowledge of community resources and AHS mental health services available within the Central Zone for child and families. Working in Connect Care is considered an asset.
