Candidates are invited to apply for the permanent, full-time position of Social Worker at Caressant Care on McLaughlin Road- 96-bed home located in Lindsey, Ontario. The home is dedicated to enriching the lives of residents in partnership with family members, valued employees and community partners

Reporting to the Executive Director, the Social Worker position is a critical role that works in collaboration with an interdisciplinary team to advocate and provide support and services to residents and family members.
As part of our team, your role as a Social Worker contributes to our resident experiences in the following ways:
- Planning, developing, implementing and evaluating individual social work services to meet the identified needs of residents and family members; ensuring those activities are aligned to the home’s strategic plan, mission and values.
- Promotes a resident-centered care approach that engages and supports residents and family members with respect to living in the home, behavioural challenges and life adjustments
- Acts as an advocate for the residents and promotes the Residents’ Bill of Rights.
- Maintains a records system which is conducive to optimal case management, in accordance with the home’s policies and legislative requirements; maintains confidentiality of information.
- Coordinates residents’ admissions to the home, transfers and re-admissions, working collaboratively with external stakeholders and partners.
- Establishes schedules and leads the resident multidisciplinary care conferences and interdisciplinary meetings as per applicable legislation and based on the residents’ needs and assessments.
- Consults with multidisciplinary team and community workers/agencies concerning the psychosocial management of residents.
- Establishes and maintains communications with other disciplines, residents and families to promote and meet person-centered resident needs.
- Collaborates and supports residents and family members with respect to the provision of external community services.
- Supports families and residents in their input in resident’s plan of care
- Participates where relevant in planning committees and staff education within the home.
- Maintains records, tracks, and evaluates complaints and concerns.
- Participates in the ongoing education and keeping up to date with the changes in the professional field.
- Completes RAI MDS as required.
- Works in accordance with the home’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
- Performs other duties as assigned.
What You Need:
- Degree in Social Work from a recognized educational institution.
- Must have a minimum of one (1) year experience in a health-care setting, preferably long-term care.
- Current membership in the Ontario College of Social Workers and Social Service Workers or eligible for membership with the Ontario Association of Social Workers or the Ontario College of Social Work.
- Must posses excellent written and oral communication skills and relationship-building abilities.
- Proficiency with Microsoft Office and other computerized documentation systems.
- Possess specialized knowledge in Gerontology, Behavioural Disorders, Complex and Palliative Care.
- All applicants must successfully pass the required Vulnerable Sector Police Check.
