SOCIAL SERVICES WORKER / PLACEMENT SERVICES COORDINATOR
Copernicus Lodge is a not-for-profit, charitable and ethno-specific organization made possible through the efforts of the Polish Canadian community and the vision of Father Michael Smith, OMI. Located in the heart of the Roncesvalles community in west–end Toronto, we are a 228 bed Long Term Care home.
Position Summary:
Reporting to the Social Worker, under the direction of the Director of Care, the Social Service Worker / Placement Services Coordinator primary role is providing support to residents and their families to achieve optimal social functioning. This role is also responsible for all aspects of the long-term care placement process within the home.
The incumbent is required to meet the needs and expectations of the residents in accordance with the Ministry of Health, Fixing Long Term Care Act’2021 and its Regulations and the organization’s mission, vision and values. The individual is accountable for working in collaboration with the other multidisciplinary departments and actively participates as a member of the multidisciplinary team. The Social Service Worker / Placement Services Coordinator is also required to demonstrate regard for the rights and responsibilities of residents and their family or substitute decision makers in matters such as privacy, beliefs, values, respect, and confidentiality.
The ideal candidate is a self-starter who has a genuine desire to work with seniors, an understanding of the Residents’ Bill of Rights and Compliance Standards as set out by the Ministry of Long Term Care and a passion for helping to support residents through counselling, advocacy and referral support.
Responsibilities:
1. Responsible for helping residents to achieve optimum social functioning.
2. Empower and advocate for residents to meet their goals and to access needed services.
3. Provide counselling, therapy and referral support to residents and families.
4. Intervene and assist with crisis situations as necessary under the guidance of the Social Worker
5. Participates in determining resident’s suitability for bed vacancy and evaluation of incoming applications.
6. Arranges for the admission, discharge or transfer of residents in the Long-Term Care Home and coordinates the LTC orientation.
7. Carries out all Long-Term Care admissions, discharges and transfers while complying with the policies and procedures of the Toronto Central LHIN, Toronto Community Care Access Center and the Ministry of Health and Long Term Care.
8. Processes and maintains records of residents of Copernicus Lodge and assists them with their specific needs as they relate to their accommodation.
9. In collaboration with the Social Worker and nursing team, the Social Services Worker/Placement Services Coordinator works as part of the multidisciplinary team and uses a wide range of highly skilled casework, group-work, and community organization techniques to:
· Assess resident and family dynamics and needs.
· Provide emotional support to residents and family members.
· Act as the resident’s liaison in the community, making referrals whenever possible.
10. Coordinates and facilitates tours to potential and newly accepted residents and their family members/POAs
11. If the Family Council requests, shall act as the Family Council assistant.
12. Acts as an advocate for residents, lobby for solutions to problems directly affecting residents
Qualifications:
· Social Service Worker Diploma
· Current registration in good standing as practicing Social Service Worker with the Ontario College of Social Workers and Social Service Workers
· Minimum of 1-2 years recent experience in long term care is preferred
· Knowledge and experience in gerontology as well as an understanding of the needs of the long term care clients
· Ability to speak Polish would be a definite asset for resident and family engagement.
· Demonstrated leadership experience and skills.
· Flexible, organized and self-directed.
· Excellent decision making, critical thinking and problem solving skills
· Demonstrated commitment to professional development and learning
· Strong computer skills including experience with an electronic patient record and ability to use Microsoft Excel and Word, Outlook and the internet
· Flexibility for shift work including working some evenings and weekends
