The Social Worker will report to the Chronic Pain Manager as part of the Chronic Pain multidisciplinary Service team. The Social Worker will provide assessment, support remediation, and options for prevention in the face of social barriers for our patients. They will serve as a catalyst to support individuals & groups to enhance social, emotional and mental health functioning by providing direct counselling services, program development, case management, consultation, health promotion, service navigation and delivery of chronic pain focused programming. The Social Worker will collaborate with the Chronic Pain Team along with respective services and community partners to support patients whose needs are both dynamic & complex helping to mitigate risk and multifaceted social consequences which may involve a high level of multi-agencies collaboration.
The Social Worker is responsible for ensuring high quality clinical & multisystemic service to the Chronic Pain Program and its patients throughout the Eastern Health Zone. The Social Worker role encompasses a versatile skill set including but not limited to mental health treatment, interdisciplinary collaboration, consultation, case management and support to patients, their families and colleagues as required.
We would love to hear from you if you have the following:
Master’s Degree in Social Work- Registered health care professional with the Nova Scotia College of Social Workers
- Minimum of two (2) years clinical experience in a chronic pain service or ability to demonstrate scope of practice within this specialty
- Minimum of three (3) years clinical experience in mental health treatment
- Experience and well-versed knowledge navigating social, political, public & healthcare systems as well as the development and maintenance of intersystem partnerships
- Comfort and Experience in program development and group facilitation both in person and through virtual platforms
- Preform activities in alignment with community health prevention & promotion as well as advocacy within our community given patient population
- Reliable transportation and Willingness to travel in order to offer versatile service delivery, when it is to the benefit of the program or optimize patient care
- Excellent communication skills; verbal and written
- Evidence of ability to monitor, adjust and evaluate care plan in collaboration with the healthcare team
- Supportive, adjustment and change-orientated counselling to address patients’ clinical needs
- Experience working in an interdisciplinary service delivery model
- Ability to work independently and as part of a team
- Ability to negotiate and advocate for resources to implement or optimize patient treatment plans
- Demonstrate interest in ongoing professional and clinical development and participation in work related committees
- Computer skills including word processing, spreadsheet and PowerPoint
- Excellent leadership and communication skills and ability to establish and maintain good working relations

$37.13 - $48.81 Hourly
