Registered Social Worker Job Description:
In this role, the Registered Social Worker will provide comprehensive assessments, and care planning and treatment coordination for designated patients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Registered Social Worker will provide direct psychosocial care, counseling support, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating patient self-determination. The Registered Social Worker will be responsible for providing social work services to maintain or improve the health and social well-being and functioning of those receiving services from STEPS.
Role/Duties:
· Support patients with identifying and applying for funding sources; Income Assistance, Persons with Disability applications, Canadian Pension Plan Disability Benefits and Employment Insurance.
· Assess eligibility for medical coverage and assist patients with registration to Fair Pharmacare.
· Support patient needs for medical equipment and other medical needs by seeking out coverage and applying for same through their private benefit plan, Ministry Benefits and/or Special Authority.
· Link patients to community services/programs where appropriate and collaborate for holistic care planning and support; MHSU, Counselling, Elizabeth Fry, Kamloops Immigration Services, Interior Community Services, Home and Community Care, and others.
· Support patients with identifying and securing housing supports and referring to community programs. For example, connecting patients with BC Housing, Indigenous Housing, Lii Michif Otipemisiwak,ASK Wellness, and other organizations as appropriate.
· Support, management, and provision of purchases for patients urgent and imminent needs.
· Work alongside the Providers and support patient needs as identified.
· Support with navigating health and clinical services.
· Providing mental health check ins, including counselling support (not counselling services).
· Develop and facilitate groups where appropriate.
· Provide psychoeducation to patient and family.
· Practice within a trauma-informed, strengths-based, person-centered approach
· Other duties as required.
Experience and Education:
· 2-5 years of customer service experience
· Bachelor of Social Work degree
· Registered Social Work designation recognized in BC
Job Type: Part-time
Part-time hours: 20-30 per week
Salary: $28.00-$40.00 per hour
