McMan is looking for a Full Time Case Manager for the Permanent Supportive Housing Program in Medicine Hat.
What is Recovery Based Permanent Supportive Housing (PSH)?
PSH is a 24/7 support that works with some of the most vulnerable in our community to provide housing stability and improve quality of life. PSH believes that housing is a right and that everyone deserves a safe and stable home. The individuals working with PSH have complex needs and have a history of homelessness or housing instability. The program utilizes trauma informed, recovery oriented and person-centered approach to supporting individuals to stabilise their housing and connect to necessary supports identified through individualized service plans.
Here’s how you will contribute as a Case Manager within the Permanent Supportive Housing Program:
- Provide ongoing assessment, care coordination and monitoring of needs of service participants
- Ensure that the case management process is logically, concisely, and legibly recorded
- Teach life skills conducive to long term housing, including but not limited to grocery shopping, house cleaning, cooking, and crisis management.
- Coordinate and/or participate in case conferences with all relevant community organizations connected to the participants as required to ensure a collaborative community of care for each participant.
- Be a part of the On-Call rotation.

Here’s what you’ll need to get an interview:
- Degree/Diploma in human services, criminal justice, or related field; a combination of education and experience may be considered.
- Understanding of Housing First philosophy, Permanent Supported Housing model, cognitive and physical disabilities, addictions, and mental health.
- Experience working with multiple stakeholders.
- Experience with participant case coordination.
- Experience working with multi-barriered individuals and diverse populations.
- Microsoft Office abilities (SharePoint, One Drive, Word, Excel, Outlook, etc.)
- Current Standard First Aid CPR Level C Certificate.
- Clear Child Intervention Record Check and Criminal Record Check with vulnerable sector dated within past six months.
- Valid Alberta Class 5 Driver’s License, Driver’s Abstract, and use of reliable vehicle with $2M Third Party Liability insurance.
