COMPENSATION: $54,488 - $70,050 Annually Prorated

To help fulfill its mission, LOFT is hiring a Coordinator of Care.
JOB SUMMARY:
The Coordinator of Care will provide support to clients of the LOFT Hub and Spoke – Penetanguishene program and the Back to Home – Penetanguishene hub. The candidate will be responsible for the intake and coordination of assisted living/supportive housing services to 40 older adults and seniors living within Penetanguishene. The position serves a diverse client population which includes frail, cognitively impaired seniors who may be living with mental health and other challenges while living in the community. The staff will provide care coordination, assess clients for service eligibility utilizing the Inter RAI CHA assessment and review collaboratively with Program Manager, the assisted living services to ensure the program meets the needs of the client within the mission, vision and values of LOFT’s program.
Note: Staff will be required to work in residents’ rooms and may therefore be exposed to secondhand smoke. In addition, this position may require weekend shifts.
WHAT YOU WILL DO:
- Being involved in assessing the individuals referred to the program, determining their daily living requirements, including personal care and nursing needs while living at the program;
- Developing a plan of support and recovery which includes identifying the priorities, goals and required supports for their transition from the program to stable housing in the community, in conjunction with other professionals and staff;
- Re-evaluating the level of support provided and interviewing residents as to their suitability for independent residential units and sites;
- Participating on case conferences and providing case management services to those who require these services;
- Liaising and acting as the main linkage with other service providers as needed;
- Establishing and maintaining a housing referral network specifically geared to meet the needs of clients;
- Providing direction and support to staff related to direct service delivery.
- Developing and implementing operating procedures for the program and ensuring compliance with established standards;
- Maintaining a general awareness of the daily activities in the program, identifying staffing issues;
- Promoting the personal empowerment of each client by building a rapport through coaching, mentoring and support, enabling each to meet the daily requirements of their living environment and directly assisting with their daily living activities, as required
- Providing 24 hour crisis management on a rotational basis through a pager or similar system;
- Mediating issues or conflicts between residents, or residents and neighbours, teaching conflict resolution; following-up on issues with residents to ensure resolution.
- Establishing and maintaining a support network of social, family, medical and psychiatric community links and resources; advocating on their behalf in navigating the social services and health care systems to ensure accessibility and availability of services
Records And Data Collection
- Maintaining accurate records and files on the provision of service to residents/clients;
- Ensuring the timely collection of all required data (CIS) and that these reports are distributed to LOFT by the required dates;
- Assisting with and ensuring assessments (e.g. RAI CHA) are up to date and completed
Leadership And Staff Oversight- Recommend and liaise with the Program Manager in the selection of all staff on the basis of job description and salary scales by LOFT Community Services guidelines
- Develop cooperative action, build staff relationships and act as a liaison between staff and residents
- Scheduling staff, completion of payroll and attendance(as designated), assigning duties
- Assisting in the selection, orientation, training and performance management of staff
- Ensure that LOFT Community Services policies and procedures are understood and reviewed annually by staff and are carried out by them, in collaboration with the Program Manager
- Develop and coordinate training and professional development programs/activities
- Collaboratively with the Manager, support the overall supervision of Program staff, including job performance appraisals as specified by LOFT Community Services policy.
- Coordinate and attend regular staff meetings and supervisory meetings as required
- Participating in the annual program planning process which includes preparing the service plan, operating plan, client satisfaction feedback process, and other research activities;
- Identifying gaps in service and recommending solutions;
- Participating in meetings, planning and evaluation initiatives, committees, events and activities;
- Compiling program evaluation reports and statistical information, inputting as required;
- Provide other duties, as assigned by Manager.
WHAT YOU BRING:
- Completion of a certificate or equivalent in Human Services (i.e. Social Service Worker, Social Worker, etc.); with eligibility for enrolment in the required regulatory body of the province of Ontario
- A minimum of three (3) years or care coordination/case management experience;
- Experience with frail, cognitively impaired older adults and seniors with complex challenges, incl. mental health, physical/medical, cognitive impairments, severe social isolation, etc.;
- An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with clients on goals/priorities;
- Excellent assessment and crisis intervention skills, with well developed problem solving abilities and interpersonal skills;
- Proficient in InterRAI CHA assessment as well as applicable computer programs; experience with OCAN (Ontario Common Assessment of Need) an asset
- Experience in responsive behaviours related to cognitive impairments, mental health, addictions, etc.
- Excellent knowledge of services available from the Toronto Central CCAC, community support providers and psychogeriatric supports;
- First Aid and CPR certification;
- Good oral and written skills; excellent computer skills
- Ability to work effectively in a team environment;
- Ability to communicate in a different language, an asset.
- Assumes other duties at the hub as appropriate
