We are actively recruiting for Director of Clinical Operations and Patient Flow
1 Year Temporary Contract
The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church& Bloor St), Harbour Lights (Queen & Jarvis Street) houses and a Specialized Care Centre (Victoria Park & Mc Nicoll). Position Summary
The Director of Clinical Operations and Patient Flow is responsible and accountable for leading the inter- professional teams in collaboration with a team of Patient Care Managers consistent with the principles underpinning Patient / Person Centered Care and in keeping with the Mission, Vision, and Values of The
Salvation Army Toronto Grace Health Centre. The Director provides leadership and oversight to ensure the efficient and effective operation of inpatient units, and patient flow and bed occupancy utilization across the various sites. The Director of Clinical Operations is a leader, coach and mentor for managers, nurses, allied health professionals and administrative staff.
Main Responsibilities
- Efficient and effective operation of several clinical inpatient units across three sites.
- Ensure the safe and effective delivery of patient care services.
- Work closely with Patient Care Managers and others to provide guidance, coaching and mentorship.
- Work in close collaboration with various stakeholders to address quality, risk, and safety concerns.
- Support planning and implementation of Quality Improvement initiatives across the organization.
- Ensure timely collection and reporting of data for Quality Improvement and Patient safety reports.
- Complete and submit reports as required to external regulatory organizations (i.e. MOH, OH ).
- Participate in several internal and external committees with other organizations within the GTA.
- Contribute to staff development by promoting and ensuring a supportive/collaborative learning environment and best practice standards for all staff.
- Report clinical performance outcomes indicators & metrics to the VP of Clinical Programs.
- Facilitate and support the development of Operational Plans that support the Organization’s strategic priorities on the various clinical areas/programs.
Qualifications and Education:
- Master’s Degree in a Health Care Related field.
- Membership within a professional association.
- Minimum 5 years of relevant management experience, and 15 years of clinical experience in acute care and or continuity complex care.
- In-depth knowledge of relevant regulations within the health care sector.
- Knowledge and strong understanding of Human Rights, ESA, OHSA, and others.
- Outstanding communication, interpersonal, emotional intelligence, and leadership skills required.
- Exceptional organizational and time management skills.
- High degree of professionalism, integrity, maturity, discretion, initiative, and judgement.
- Ability to establish and expand professional relationships and network with internal and external stakeholders.
- Proven change management skills combined with the ability to work in close collaboration with others.
- Demonstrated problem solving and conflict resolution skills.
- Proven ability to promote and support academic practice environment within the clinical setting.
- Self-motivated, team player, and ability to thrive in a fast-paced environment.
- Client service oriented, with the ability to effectively work within a framework that supports diversity, inclusiveness, and equity.
