Position Title: Temporary Nurse Practitioner RN (EC) Primary Care
(0.6 FTE – 1.0FTE dependent on applicant availability)
Duration: 3 months with possibility of extension
Reports to: Clinical Director
Unionized Position - Compensation: As per ONA Collective Agreement.
The Harrow Health Centre (HHC) is a dynamic group of physicians, nurses and other health care professionals. Our mission is to work together with our community to achieve excellence in the provision of health care by ensuring quality, responsiveness, accessibility, effectiveness, productivity and continuity of health care.
The HHC provides programs and services for rostered patients in Harrow and surrounding communities. The NP will function as an effective member of the family practice team and provide on-going primary care to patients who include the individual throughout their lifespan, the family, and the community, with a strong emphasis on health promotion, patient education and disease prevention to the full scope of their practice.
The NP will function as a member of the inter-professional care team responsible for providing primary health care. The NP will practice within their full scope of practice and abide by the Practice Standards and Practice Guidelines as outlined by the College of Nurses of Ontario.
The NP will be accountable to the Clinical Director and the College of Nurses of Ontario for the standard and quality of service provided in their clinical responsibilities. Ultimately the NP is responsible to the Executive Director of the Harrow Health Centre Inc.
Responsibilities:
· Provide primary health care services within the scope of the RN, EC including: physical assessments, treatment, diagnosis and interpretation of test results; therapeutics; health promotion/education/counselling; history taking, screening, referral, and follow up.
· Consult with other members of the healthcare team as needed, and work in partnership with other team members to share responsibility for the planning, provision and co-ordination of patient care.
· Works within medical directives as established and approved by the HFHT
· Provide services during operational hours of the Harrow Health Centre.
· Provide services, as necessary, at Harrowood Seniors Community and McGregor Satellite site.
· Work within a primary health care framework that emphasizes disease prevention and health promotion and education.
· Recognize the special risk factors (social, physical, psychological etc.) which may affect the health of patients.
· Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records using the electronic health record (Accuro), participating in chart reviews and discussing problem cases.
· Document using EMR refers to and arranges follow-up as necessary
· Participate in administrative activities of the health centre by:
- providing input into program development,
- delivery and evaluation;
- collecting statistical data as required by the centre;
- identifying areas of development with regards to policies, procedure and protocols;
· Participating in staff, team and other committees; and, liaising with other institutions, organizations and health and social service professionals.
Qualifications:
Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario.
Current registration with RNAO and evidence of liability insurance
Experience and proficiency in current physical assessment and treatment methods within the scope of practice of the Nurse Practitioner-Primary Health Care
Experience working in an electronic medical records environment; Experience with Accuro EMR is an asset
Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient
Demonstrated ability to work effectively in a multi-disciplinary environment.
Demonstrated commitment to and knowledge of community- based health care.
Awareness of and sensitivity to the health issues of a diverse community
Excellent communication, interpersonal, organizational and administrative skills including computer literacy
Ability to work extended hours; possibility of at least 1 evening shift per week
