NURSE PRACTITIONER, HIV Program
1.0 FTE - (5 days)
WHIWH CHC provides racialized women, trans and non-binary clients from the African, Black, Caribbean, Latin American, and South Asian communities in Toronto and surrounding municipalities with culturally safe, relevant, and responsive primary healthcare. We are changemakers engaged in knowledge transfer and exchange, research, capacity building, and advocacy within and across multiple systems.
The Centre has an opportunity for a professional, motivated and dynamic individual interested in working with women living with HIV to join the team.
RESPONSIBILITIES:
1. Provide comprehensive HIV Primary Care
2. Perform excellent clinical assessments combined with sound clinical judgment/diagnostic and treatment skills – within full scope of practice in accordance with the College of Nurses of Ontario (CNO) standards of practice for Registered Nurses in the Extended Class (NPs).
3. Support pregnancy planning for women living with HIV
4. Perform initial prenatal assessments prior to referring to Obstetrician and Midwifery
5. Support access to a cascade of services inclusive HIV treatment, support and care including adherence and retention in care
Complete timely and accurate medical records of client encounters, documentation and third-party correspondence using the Centre’s Electronic Medical Record (EMR) system.
Contribute to activities related to the development, implementation and evaluation of medical directives, policies and procedures, best practices and clinical protocols
Participate in the development, implementation, monitoring and evaluationof treatment, education, counselling and health promotion programs and services related to HIV for individuals, groups and the communities.
9. Participate in chart reviews, case conferences and accreditation activities
May provide clinical supervision and training for NP students.
Ability to work a flexible schedule with at least one evening per week.
12. Perform other duties as assigned that are reasonable with scope of practice.
QUALIFICATIONS:
1. Undergraduate degree in nursing from a recognized university
2. Registration in the Extended Class with the College of Nurses of Ontario
3. Hold a Prescribing Narcotics and Controlled Substances certificate endorsed by the CNO.
4. Specialization (or interest to specialize) in HIV treatment, support and care and related issues
5. Three to five years of nursing experience in a community setting or a combination of community and hospital or public health settings; experience in a primary care setting, an asset.
6. Demonstrated critical thinking and problem-solving skills, organizational and interpersonal skills with an ability to work effectively and respectfully in a collaborative, inter-professional team environment.
7. Experience in program development, implementation, monitoring and evaluation.
8. Knowledge of women’s health issues.
9. Knowledge of HIV and its impact on racialized women
10. Experience in prenatal care is an asset
11. Proficiency in the use of computers and various software applications.
12. In addition to English, knowledge of a language spoken within one of the four priority communities, an asset.
