Job Title: Community Integration Coordinator

Position Scope:
Reporting to the Director of Disability Support, the Community Integration
Coordinator is responsible for the delivery of services designed to support clients who are preparing to transition into community living, supporting them to make their own decision on the community they want to live in and who they want to live with.
Primary Responsibilities / Duties: a. The development of community transition plans for all participants that fall under the
Mountains and Meadows Care Group umbrella. b. Prepares participants for the transition from congregate living to living in the community. c. The development and implementation of programs that meet the needs of the participants based on identified goals such as maintaining a household including home safety, laundry, shopping and banking, budgeting, money management, preparing meals, transportation for accessing community services such as stores, recreational activities, and medical appointments, participating in educational, leisure, volunteer, or work activities. d. Works collaboratively with other organizations to ensure that participant’s training needs are being met. e. Works collaboratively with participants to support vocational placements and ensure current placements are meeting the needs of the individual. f. Support opportunities for social, leisure and recreation in the community that supports an individual’s own goals for community inclusion. g. Supports the fostering of community connections and resources as well as maintaining and growing family/friend connections. h. Attends all participant support plan meetings with the participant and supports the participant to provide updates on achievement of agreed goals. i. Works collaboratively with all stakeholders including Care Coordinators. j. Provides evidence-based outcomes clearly showing that needs are being met. k. Provide education to participants and staff on consent, relationships and supportive decision making.

DCS. m. Other duties as assigned.
Minimum Education & Qualifications: a. Bachelor’s degree in Social Work, or related field with applicable experience b. Must be educated in the Supportive Decision-Making Framework and the UN Convention of Human Rights. c. Basic knowledge of such software as MS Access, MS Excel, MS Word and other software. d. Ability to use social media platforms effectively. e. Experience (2 years) in working with individuals with different abilities. f. Must possess a valid Nova Scotia’s license, insurance as per the policy of the organization and be willing to transport clients using own vehicle. Must provide a driver’s abstract upon hire.
Special Knowledge & Skill: a. Must be able to work independently with minimum supervision. b. Must be able to maintain effective relationships with all stakeholders. c. Must have knowledge of individuals with different abilities and experience working in this area.
Judgement & Initiative Required: a. Must show good judgement in any circumstances that may arise at the Meadows and community programs or in our communities. b. Must have the initiative to work with, and support individuals and staff who support all programs under the Meadows and Community.
Physical Demands:
Must be in good mental and physical health and able to bend, lift, and stretch as required for job performance. Position involves standing, walking, sitting and use of a computer.
Working Environment:
Ability to work in environments that may have plants and animals as well as working out of doors The MMCG provides services for adults who may have physical limitations, intellectual disabilities, mental health concerns or dementia who, from time to time when upset, may exhibit behaviors.
This position requires some evening, weekend and holiday work depending on program functions.
