Job Description
We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, Pathways to Education, infant and child development programs, parenting programs, youth outreach and employment services.
Position Summary:
The Child and Youth Social Worker provides individual assessment, time limited, short term individual and group counselling, group work ,case management and coordination, and where appropriate, systemic advocacy for parents and children in the and Children Youth and Families programs. The goal of the program is to support prenatal, early years, middle years, adolescents, young adults and their families.
Responsibilities Include:
Client Assessment:
- Assess client status and service needs;
- Jointly identify service needs and goals with the client and other members of the Early Years team;
- Provide feedback to referral source as required.
- Counseling and Referrals:
- Work with the client to develop a plan for appropriate services, sensitive to client’s issues;
- Provide mutually agreed upon time limited and short term, individual counseling services;
- Utilize a variety of techniques and approaches including Dialectic Behavioural Therapy (DBT), Cognitive Behavioural Therapy (CBT), Mindfulness and/or Brief therapy;
- Provide time limited group counselling sessions;
- Collaborate with the clients on their identified issues, plans for service delivery, and appropriate referrals;
- Work with involved significant others/family, and provide referrals as directed by the client;
Program Development and Small Group Work:
- Contribute to the overall goals of the Centre by working with other staff in designing and implementing programs and services;
- Develop or assist in developing program(s) to meet needs identified by clients and/or the community by assessing needs, planning and delivering programs, and /or linking with existing programs;
- Develop and deliver small group programs such as stress management and self-care for parents;
- Conduct and complete evaluations of small group sessions and other programs.
- Liaising/Networking with Community Groups and Organizations:
- Establish and maintain positive links with relevant organizations and groups in the community;
- Work with community groups and organizations to identify service gaps and needs in the community;
- Network, liaise and jointly advocate with organizations and community groups addressing issues identified by clients and/or the community.
- Preparing Documentation:
- Ensure accurate clinical documentation including assessment notes, progress notes and correspondence are completed and maintained safeguarding confidentiality, including required consents for disclosure;
- Keep Electronic Medical Records up to date in a timely manner
- Complete and provide statistical data as required.
Qualifications and Skills required:
- Must be registered and in good standing with the Ontario College of Social Workers
- MSW preferred, or equivalent education and experience in health/social services
- At least 3 years’ experience doing similar work
- Prior experience working with children, youth and young adults
- Experience in clinical note taking and using electronic medical records, EMR, (PS Suites, an asset)
- Experience in Infant Mental Health and children and youth with special needs is an asset
- Experience working with high- risk families, racialized communities, child development and child welfare
- Demonstrated experience with trauma informed work and interventions such as DBT, CBT, Mindfulness, Brief therapy, play therapy, harm reduction etc.
- Demonstrated problem-solving, time management and networking skills
- Group facilitation and counselling skills
- Knowledge of health promotion, advocacy, program planning, research and evaluation
- Ability to work collaboratively within and across teams
- Ability to work effectively with people of various cultural, age and economic backgrounds
- Second language is an asset
- Understanding of and commitment to Anti-oppression principles and practices
- Knowledge of the Regent Park area and issues affecting families living within the Regent and Moss Park communities
- Must have up to date COVID-19 vaccination for the protection of clients and staff.
