POSITION: Nurse Practitioner MRP
DEPARTMENT: Patient Care (Sub-Acute ALC)
CONDITIONS: Full-time
SHIFTS:Days (Evenings / Weekends / On-Call, as required)(subject to change)
WAGE RANGE:Scale Minimum MaximumONA06 $53.45/hr $60.97/hr
The Nurse Practitioner (NP) is a masters prepared nurse that is registered with the College of Nurses of Ontario (CNO) in the Extended Class with an Adult or Primary Health Care specialty certificate. The Nurse Practitioner demonstrates advanced knowledge, skill, and judgment in providing care to patients that is consistent with CNO Practice Standards for Nurse Practitioners (2021) and the competencies defined by the Canadian Nurses Association Competencies for Nurse Practitioners (2010).
The Nurse Practitioner MRP is functioning in an expanded role as Most Responsible Provider (MRP) to post-acute patients. The NP MRP will practice autonomously, within legislated scope of practice, collaborating as necessary with physician partners, to optimize patient outcomes and enhance safety, efficiency, effectiveness, and accessibility to care.
Collaborate with other nurse leaders to support the development of nursing staff and the implementation of key nursing practice initiatives, including the development, implementation, and evaluation of nursing best practice guidelines. Collaborate with physicians and other interprofessional members to assist in developing, applying, and evaluating organizational polices and standards of care for the sub-acute population. Collaborate and plan with the interprofessional team to deliver holistic patient centered care to patients and families across the inpatient continuum.
RESPONSIBILITIES:Clinical Domain- Provide clinical care, as the MRP, to patients and their families comprises the central part of the NP role. This role is responsible for Alternate Level of Care (Subacute) patient population
- Demonstrate the following competencies:

- Responsible for supporting discharge planning and discharging patients from designated areas

Education / Knowledge Translation- Uses knowledge-transfer (knowledge translation) techniques in innovative ways to put research-based knowledge into daily practice and to support and educate interprofessional team members
- Provides formal and informal teaching and coaching in the management of clinical care to interprofessional team members, serving as a resource person, educator and role model
Quality Improvement and Research- Acts as leader to contribute to the culture of innovation at QCH by advancing and/or appraising nursing research, practice guidelines, and current best practices
- Evaluates outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends
- Designs, directs, and evaluates quality improvement methodologies and/or research to promote safe, timely, effective, efficient, equitable, and patient-centered care with interprofessional team members
Leadership
- Supports the development of interprofessional team members and supports the implementation of key nursing practice initiatives, including the implementation of nursing best practice guidelines
- Mentors nurses and student NPs to facilitate professional growth
- Acts as a transformational leader by using ideals, inspiration, and intellectual stimulation to influence the behaviors and attitudes of others towards the goal of excellent patient care
REQUIREMENTS:- Master’s degree in Nursing
- Registration with the College of Nurses of Ontario (CNO) in the Extended Class (RN-EC)
- NP Adult preferred, or Primary Health Care NP Certificate with relevant hospital experience
- Requires at least five (5) years of relevant experience including three (3) years working in an acute care centre
- Experience in working in an advanced clinical practice role preferably as a Nurse Practitioner or Clinical Nurse Specialist
- Experience in nursing research methodology
- Membership in Nurses Association of Ontario (RNAO) and Nurse Practitioners Association of Ontario (NPAO)
- Current Certification in Advanced Cardiac Life Support
- Additional education/certification and experience in Geriatrics would be an asset
