OVERVIEW
As an integral member of an interprofessional team, the Social Worker for the Mental Health Services cares for individuals with mental illness. The Social Worker has primary therapist responsibilities providing individual and group counselling, therapy and support using a holistic approach that is trauma informed, gender transformative and anti-oppressive. This position will also focus on younger adults accessing mental health care in the outpatient department.
The Social Worker will provide comprehensive psychosocial assessment and formulation of treatment planning and interventions to adult and young adult patients with serious mental illness and their families. Case management; care coordination; time-limited individual supportive counselling and group treatment; locating and arranging community resources, linking with family and community services are important components of the Social Work role. The Social Worker participates in program planning, implementation, and evaluation for other program initiatives.
EDUCATION
- Master of Social Work from an accredited school of social work, or equivalent degree, as recognized by the Ontario College of Registered Social Workers and Social Service Workers (OCSWSSW)
- Registration licensure with a regulatory college allowingindependent practice of the Act of Psychotherapy (eg., Ontario College of Social Workers and Social Service Workers)
QUALIFICATIONS
- Minimum of 5 years of recent experience working with adults living with mental health challenges, substance use/addictions and concurrent disorders in an outpatient mental health environment
- Experience working with transitional-aged youth, young adults and their families in a therapeutic setting required.
- Experience using a range of therapeutic modalities that are client-centred and recover-based.
- Extensive knowledge of / and experience providing services with a Motivational Interviewing, Cognitive Behavioural Therapy focus and Solution-Focused techniques required.
- Proven competence and recent experience in case management, therapy/counselling, care planning and group facilitation in an outpatient setting.
- Thorough knowledge of, and proficiency in, current best practice assessment and counselling techniques for individuals, families and groups.
- Training knowledge and experience in anti-oppressive, gender transformative approaches to care/treatment.
- Demonstrated competency andexperience working with diverse communities using a health equity lens.
- Knowledge and experience applying conflict resolution and de-escalation techniques.
- Experience with preventing and responding to crisis.
- Sound knowledge and experience working with adults and young adults with histories of trauma, abuse, self-harm and/or suicidal behaviour.
- Experience delivering care via virtual platforms.
- Knowledge of / and experience with youth with mental health challenges, preferred, to assistin program coverage.
- Demonstrated knowledge of related legislation.
- Knowledge of community adult, young adult mental health and substance use/addiction resources.
- Demonstrated ability to work in an interprofessional team environment.
- Strong oral and written communication skills.
- Excellent interpersonal skills.
- Proficiency in the use of computer technology and various software applications.
- Good work and attendance record required.
