Position Summary
A world leader in neural and sensory science, Toronto Western Hospital has a strong community and population health program, providing innovative programs for and information to seniors, diabetes patients, and many multicultural groups. Toronto Western Hospital has been serving the health care needs of its culturally diverse local community for more than 100 years. Today, the hospital provides highly specialized tertiary care to people from surrounding areas and across Canada.
The Social Worker will be an integral member of the Bariatric Program and the Allied Health Department. Bariatrics is an interesting and expanding area of practice within the health care system. As this is a relatively new program at Toronto Western Hospital, this position would be ideally suited for an experienced Social Worker with a passion for program development. The Social Worker will work as integrated member of the inter professional care team.
Duties
- Provide pre-operative assessment and education, peri-operative support and post-operative support and follow-up
- Function at a high level of professional autonomy
- Demonstrate expert skills to formulate clinical decisions, and patient-centred care for bariatric patients
- Demonstrate commitment to continued professional development, critical decision making and patient-centered care
- Perform other duties consistent with the job classification, as required
Qualifications
- Successful completion of a Masters Degree in Social Work (M.S.W) or recognized equivalent required
- Membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
- Experience with psychiatric assessment and familiarity with DSM Classification
- Experience providing psychoeducational intervention; cognitive behavioural therapy; motivational enhancement therapy
- Experience with crisis intervention
- Experience with group based interventions
- Experience in assisting patients/clients to affect personal change
- Commitment to patient-centred care with the ability to adapt to a diverse patient population
- Excellent interpersonal, verbal and written communication skills
- Demonstrated ability to work effectively as an inter professional health care team member
- Excellent organizational, time management and problem solving skills
- Membership with Ontario Association of Social Workers preferred
- Two (2) years experience working with a multi-disciplinary team in a health care setting preferred
- Experience in an ambulatory patient care environment preferred
- Weight management counselling experience/ experience with Bariatrics preferred
- Experience in educating health professionals and social work interns preferred
- Program planning and evaluation and interest/experience in research preferred
