Job Description
The successful applicant will be part of a multi-disciplinary team providing care to patients on the Rehab and Complex Continuing Care units.
Accountabilities:
- Demonstrate professional accountability, strong teamwork skills, and a willingness to participate quality improvement projects related to the unit and the organization
- Will report to the Clinical Services Manager for Complex Continuing Care, and will support the needs of the Social work team within William Osler Health System
Qualifications
- Masters degree in social work required
- Three to five years experience working with hospital or related experience
- Knowledgeable in community resources
- Eligible for OCSWSSW membership
- Knowledge and experience working with relevant legislation (i.e. SDM Act, POA, Consent and Capacity, etc.)
- Knowledge of the psychosocial impact of chronic illness on the person and the family
- Experience with MDS and FIM scores
- Experience with post-stroke population
- Excellent communication and interpersonal skills
- Ability to work as part of a multi-disciplinary team
- May be required to travel between and work at all Osler sites
- Excellent organization skills and ability to set priorities as required
- Must demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and Collaboration
- Good critical thinking and problem-solving skills required
- Excellent attendance, punctuality and work record
- Computer proficiency required
Additional Information
Hours: Currently Days, 8 hour shifts, including weekends and statutory holidays (subject to change in accordance with operational requirements)
